How to become a Hi-Tech ISP customer through FHTM Canada

• Download and fill out the Hi-Tech ISP Application.
  
• Before signing the application, make sure you have thoroughly read and understand the Terms and Conditions.
  
• Download and fill out the FHTM Canada Customer Agreement Form.
  
• Once you have read the Terms and Conditions and completed both forms, fax them to the US home office at 859.422.7037.
  
• Once you have submitted those forms, call Tech Support at 877.526.7868 and a technician will walk you through the installation process.
